Questions concerning the cost of infertility treatment should be addressed in the initial stages of conversation with our staff. In addition, we recommend verification of your infertility treatment insurance benefits before your first appointment. If your insurance company requires preauthorization, we will submit this for you. You can receive free verification of your insurance benefits by following these steps:

- Allow at least one week for a response

Cost and Fees
The cost of infertility treatment will depend on the treatment plan your physician recommends for you and your insurance coverage. We are happy to discuss our typical infertility treatment fees with you. Cost will range from initial workup and evaluation fees to high-tech procedures such as in vitro fertilization (IVF), including egg donation. You are responsible for the timely payment of copayments and account balances.
Your insurance is a contract between you and your insurance company.* Keep in mind that you can be your best advocate by thoroughly understanding your policy and coverage, hopefully eliminating surprises down the road. As noted above, we recommend verification of your infertility treatment insurance benefits before your first appointment by downloading this form and faxing to our office.
If you’re verifying coverage on your own by phone with your insurance company, remember to document the reference number for the call as well as the name of the member service representative. You may want to consider the following:
- Do I have coverage for infertility treatment? For intrauterine Insemination (IUI)? For in vitro fertilization (IVF)?
- Do I have a deductible?
- What percentage of the cost does my plan cover?
- Do I need to enroll in any oversight or care management program?
- Is there a dollar limit associated with infertility treatment coverage?
- Is there a limit on the number of cycles covered?
- Is there a lifetime maximum for treatment?
- Are there any diagnosis limitations?
As a courtesy, if you’re unable to verify your insurance coverage before your first appointment, we will contact your insurance provider on your behalf and send you a letter stating their response regarding your coverage.
* Our practice does not participate with the Affordable Care Act or any Medicaid or Medicare plans.
Our center contracts with all major insurance plans. We accept, but are not limited to, Aetna, Anthem, BC/BS, Humana, Medical Mutual, Cigna, Great West and United Health Care, along with various other commercial/third-party payers. We are considered in network with most all insurance companies. Our participation with your insurance plan does not guarantee coverage for our services.
If you have a non-participating insurance company, we will require payment upfront. Payment will be expected at the time of service for all services. We do not participate in Medicare, Medicaid or CareSource.
We accept all forms of payment. You can make payment with cash, check or credit card – Visa, MasterCard, American Express and Discover. You can make payment directly at our office or by phone with our Billing Department by calling 513-853-4720.

The WINFertility Program includes both advanced medical treatment and fertility medications in a single, discounted “bundled” price for each cycle.
The Compassionate Care Program offered by Fertility Lifelines is designed to provide eligible patients savings on various fertility medications, depending on income. Eligible patients may save 25%, 50% or 75% off the self-pay price of medicine or receive a 10% discount per unit, via a mail-in rebate. We encourage every patient to apply regardless of income level or insurance benefit.
